THE INVESTIGATION
Investigation
- Gather the necessary facts, data & evidence.
- Analyse the facts, data & evidence.
- Convene & conduct a formal internal inquiry.
- Determine the Root Cause of the incident.
- Make recommendations to the employer.
- Implement the internal findings.
- Provide the findings to the Inspector (If applicable).
- Participate in the OH&S Inspector’s inquiry (if applicable).
- Review policies, procedure & practices.
- Implement Remedial actions.
- Retrain if necessary.
