Investigation

• Gather the necessary facts, data & evidence.
• Analyse the facts, data & evidence.
• Convene & conduct a formal internal inquiry.
• Determine the Root Cause of the incident.
• Make recommendations to the employer.
• Implement the internal findings.
• Provide the findings to the Inspector (If applicable).
• Participate in the OH&S Inspector’s inquiry (if applicable).
• Review policies, procedure & practices.
• Implement Remedial actions.
• Retrain if necessary.
